memory exception between Windows and a process running. What I mean by that is that Windows will lose track of it's own userspace and give the same block of memory to two different programs. Never used to see that at the dos prompt...

that would be the most common way right there, can even be caused by simple fragmentaion (bluescreen) / iexplorer.exe has caused a general protection fault, otherwise, can be bad settings in the registry as well (active x typicaly)
